Veterans' Medallion Available for Order - New Option for Marking Veterans' Graves in Private Cemeteries
 
WASHINGTON - Secretary of Veterans Affairs Eric Shinseki announced today that the
Department of Veterans Affairs (VA) is offering bronze medallions to attach to existing,
privately purchased headstones or markers, signifying a deceased's status as a Veteran.
 
"For Veterans not buried in a national or state Veterans cemetery, or those without a government
grave marker, VA is pleased to offer this option that highlights their service and sacrifices for our
country," said Secretary Shinseki. 
 
Each service has own medallion.
 
The new item can be furnished instead of a traditional government headstone or marker for Veterans
whose death occurred on or after Nov. 1, 1990, and whose grave in a private cemetery is marked
with a privately purchased headstone or marker.
 
Under federal law, eligible Veterans buried in a private cemetery are entitled to either a government-furnished
grave marker or the new medallion, but not both.  Veterans buried in a national or state Veterans cemetery
will receive a government headstone or marker of the standard design authorized at that cemetery.
 
The medallion is available in three sizes: 5 inches, 3 inches and 1inches in width.  Each bronze medallion
features the image of a folded burial flag adorned with laurels and is inscribed with the word "Veteran"
at the top and the branch of service at the bottom.
 
Next of kin will receive the medallion, along with a kit that will allow the family or the staff of a private
cemetery to affix the medallion to a headstone, grave marker, mausoleum or columbarium niche cover.

VA is currently developing an application form for ordering the medallion.  Until it is available, applicants
may use the form for ordering government headstones and markers, VA Form 40-1330

Instructions on how to apply for a medallion are found on the VA Web site at www.cem.va.gov/hm_hm.asp.
 
Veterans with a discharge issued under conditions other than dishonorable, their spouses and eligible
dependent children can be buried in a VA national cemetery.  Other burial benefits available for all
eligible Veterans, regardless of whether they are buried in a national cemetery or a private cemetery,
include a burial flag, a Presidential Memorial Certificate and a government headstone or grave marker.
 
The new medallions will be available only to Veterans buried in private cemeteries without a government
headstone or marker.  Families of eligible decedents may also order a memorial headstone or marker
when remains are not available for interment.
 
VA operates 131 national cemeteries in 39 states and Puerto Rico and 33 soldiers' lots and monument sites. 
More than 3 million Americans, including Veterans of every war and conflict -- from the Revolutionary
War to the current conflicts in Iraq and Afghanistan -- are buried in VA's national cemeteries on more
than 19,000 acres.

on the Internet at www.cem.va.gov   or by calling VA regional offices toll-free at 1-800-827-1000.

Alameda County: Remembers Our Fallen

Lance Cpl. Aaron Boyles of Alameda, California
At the age of 24 was killed September, 24th, 2004 by hostile fire in
Anbar province Iraq. He was serving with the Marine Headquarters and Service Company,
7th Marine Regiment, 1st Marine Division, 1st Marine Expeditionary Force, Marine
Corps Air Ground Combat Center based in Twentynine Palms, California

Spec. Michael T. Manibog, of Alameda, California
Died February 8th, 2008 in Operation Iraqi Freedom when his vehicle encountered a makeshift
bomb in Jaji Iraq.  Spec Manibog was assigned to the Army 1st Battalion, 21st Infantry
Regiment, 2nd Stryker Brigade Combat Team, 25th Infantry Division, Schofield Barracks, Hawaii.

Sgt. 1st Class Merideth L. Howard, of Alameda, California
Died at the age of 52 years old on September 8th, 2006 in Operation Enduring Freedom when
a vehicle-borne makeshift bomb exploded near her Humvee in Kabul, Afghanistan.  Sgt 1st Class
Howard was assigned to the Army Reserve 405th Civil Affairs Battalion,
U.S. Army Reserve,
Fort Bragg, North Carolina.

Staff Sgt. Sean D. Diamond of Dublin, California
Was killed on February 15th, 2009 at the age of 41 years old when a makeshift bomb detonated
near his vehicle in Salam Iraq during Operation Iraqi Freedom. Staff Sgt. Diamond served with the
Army 610th Engineer Support Company, 14th Engineer Battalion, 555th Engineer Brigade,
Fort Lewis, Washington.

Spec. Cheyenne C. Willey of Fremont, California
Died at the age of 36 years old on December 23rd, 2005 in Operation Iraqi Freedom when
a makeshift bomb exploded near her Humvee in Baghdad, Iraq.  Spec. Willey served with the
Army Reserve 351st Civil Affairs Command, U.S. Army Reserve, Mountain View, California.

Died on April 6th, 2004 in Operation Iraqi Freedom at the age of 19 years old, killed by enemy
fire in Anbar Province, Iraq.  Lance Cpl. Layfield served with the Marine 2nd Battalion, 4th Marines,
1st Marine Division, 1st Marine Expeditionary Force, Camp Pendleton, California.

Spec. Avealalo Milo of Hayward California

Died at the age of 23 years old on October 4th, 2007 in Operation Iraqi Freedom, killed by
insurgents who attacked his unit using small arms fire in Baghdad, Iraq.  Spec. Avealalo served
with the Army 2nd Squadron, 2nd Stryker Cavalry Regiment, 2nd Stryker Brigade Combat Team,
1st Armored Division, Vilseck, Germany.

Killed on June 5th, 2007 at the age of 28 years old during Operation Iraqi Freedom from wounds
suffered when his unit came in contact with enemy forces using small arms fire.  Sgt. Higgins
served with the Army 5th Battalion, 20th Infantry Regiment, 3rd Brigade, 2nd Infantry Division
(Stryker Brigade Combat Team), in Fort Lewis, Washington.

Infantry Division. Fort Benning, Georgia.

Died on December 5th, 2003 at the age of 20 years old by a makeshift bomb in
Baghdad, Iraq.  Spec. Clark was assigned to the Army 440th Signal Battalion, 22nd Signal
Brigade, V Corps, Darmstadt, Germany.

Died on April 15th, 2007 at the age of 18 years old.  KIlled during Operation Iraqi Freedom by enemy
small arms fire in Baghdad, Iraq.  Pfc. Walberg served with the Army 1st Squadron, 4th Cavalry
Regiment, 4th Infantry Brigade Combat Team, 1st Infantry Division, Fort Riley, Kansas.



Contra Costa County: Remember Our Fallen

Staff Sgt. Daniel R. Scheile of Antioch, California
Died at the age of 37 years old on September 23, 2005 in Operation Iraqi Freedom from injuries
sustained when a makeshift bomb exploded near his armored personnel carrier in Baghdad Iraq.
Served with the Army National Guard 1st Battalion, 184th Infantry Regiment, 3rd Infantry Division,
Army National Guard, Oakdale, California.

Sgt. John D. Aragon of Antioch, California
Died at the age of 22 years old on June 12th, 2008 in Operation Iraqi Freedom from wounds
suffered when his vehicle encountered a makeshift bomb in Khadamiyah, Iraq.  Sgt. Arogan
was assigned to the Army 1st Squadron, 75th Cavalry Regiment, 2nd Brigade Combat Team,
101st Airborne Division.

Lance Cpl. Erick J. Hodges from Bay Point, California
At the age of 21 years old Lance Cpl. Hodges died on November 10th, 2004 during the
U.S. Assault on Fallujah, Iraq.  Assigned to the Marine 3rd Battalion, 5th Marine Regiment,
1st Marine Division, 1st Marine Expeditionary Force, Camp Pendleton, California.

Pfc. Scott G. Barnett from Concord, California
Died at the age of 24 years old on January 28th 2010 in Operation Iraqi Freedom. 
Pfc. Barnett served with the 412th Aviation Support Battalion, 12th Combat Aviation Brigade,
Katterbach, Germany.  Killed in Tallil, Iraq, of injuries sustained while supporting combat
Operations.

Army Capt. John L. Hallett III, from Concord California
Died in southern Afghanistan of wounds suffered at the age of 30 on August 25th, 2009 when
enemy forces attacked his vehicle with an improvised explosive device.
Capt. Hallet III was
assigned to the Army 1st Battalion, 17th Infantry Regiment, 5th Stryker Brigade, 2nd Infantry Division,
Fort Lewis, Washington.

At the age of 20 years old Pfc. Zieske died on May 3rd, 2006 when a makeshift bomb exploded
during a dismounted patrol in Kirkuk Iraq.  Assigned to the Army 1st Battalion, 327th Infantry
Regiment, 1st Brigade Combat Team, 101st Airborne Division (Air Assault), Cambell, Kentucky.

At the age of 22 years old died on December 31st, 2008 in Operation Iraqi Freedom of
wounds suffered when insurgents attacked his unit with indirect fire in Ghazalya Iraq.  Pfc. Tollefson
was assigned to the Army Special Troops Battalion, 2nd Heavy Brigade Combat Team, 1st Infantry Division, Fort Riley, Kansas.

1st Marine Division, 1st Marine Expeditionary Force, Camp Pendleton, California.

wounds sustained when his patrol was attacked by direct fire from enemy forces in Aranus,
Afghanistan. Spec. Sangevin served with the Army 2nd Battalion, 503rd Airborne Infantry Regiment,
173rd Airborne Brigade Combat Team, Vicenza, Italy.

Capt. Bruno G. De Solenni of Crescent City, California
Died on September 20th, 2008 in Operation Enduring Freedom in Kandahar, Afghanistan of wounds sustained when a makeshift bomb detonated near his vehicle. Capt. Solenni was assigned to the Army National Guard, Joint Forces Headquarters, Element Training Team, Oregon Army National Guard.




El Dorado County: Remembers Our Fallen

Pfc. Mathew D. Taylor of Cameron Park, California
At the age of 21 years old, Pfc. Taylor died on September 26th, 2007 in San Antonio of wounds
sustained when a makeshift bomb exploded near his vehicle on July 23rd in Sarobi District,
Afghanistan.  Assigned to the Army 1st Battalion (Airborne), 503rd Infantry Regiment, 173rd
Airborne Brigade Combat Team, Vicenza, Italy

Lance Cpl. Brad S. Shuder of El Dorado, California
Died on April 12th, 2004 by hostile fire in Falujah, Iraq
at 21 years old.  Lance Cpl. Shuder was
assigned to the Marine 2nd Battalion, 1st Marine Regiment, 1st Marine Division, 1st Marine
Expeditionary Force, Camp Pendleton, California.

Sgt. Kyle Dayton of El Dorado Hills, California
At the age of 22 years old was killed in Operation Iraqi Freedom from injuries suffered from a
non-combat related incident in Ashwah Iraq.  The incident is under investigation.  Sgt. Dayton
was assigned to the Army 2nd Battalion, 504th Parachute Infantry Regiment, 1st Brigade Combat
Team, 82nd Airborne Division, Fort Bragg, North Carolina

Died at the age of 25 years old on April 7th, 2008 in Operation Iraqi Freedom, killed when his
vehicle encountered a makeshift bomb in Baghdad, Iraq.  Sgt. Smith served in the Army 4th
Brigade Special Troops Battalion, 4th Brigade Combat Team, 10th Mountain Division
(Light Infantry), Located at Fort Polk, Louisiana.

Sgt. Harvey E. Parkerson III of Orland, California
Died at the age of 27 years old on August 18th, 2004 during Operation Iraqi Freedom, killed by
enemy action in the Anbar Province of Iraq.  Sgt Parkerson III served with the Marine Battalion
Landing Team 1/4, 11th Marine Expeditionary Unit (Special Operations Capable),
Camp Pendleton, California.

Spec. Thai Vue from Willows, California
Died at the age of 22 years old on June 18th, 2004 in Operation Iraqi Freedom, killed in Baghdad
when a mortar round hit the motor pool where he was working.  Spec. Vue served with the
Army 127th Military Police Company, 709th Military Police Battalion, 18th Military Police Brigade,
V Corps, Hanau, Germany.




Humboldt County: Remembers Our Fallen

Cpl. Peter W. Schmidt of Eureka, California
At the age of 30 years old was killed on November 13th, 2007 when a makeshift bomb detonated
during dismounted combat operations in Mukhisa Iraq. Cpl. Schmidt was assigned to the
Army 2nd Battalion, 23rd Infantry Regiment, 4th Stryker Brigade Combat Team, 2nd Infantry Division,
Fort Lewis, Washington.

Capt. Andrew David La Mont of Eureka, California
Died on May 19th, 2003 during operation Iraqi Freedom at the age of 31 years old when his
CH-46 Sea Knight Helicopter went down shortly after takeoff in the Shat Hillah Canal. Capt. La Mont served with the Marine Medium Helicopter Squadron 364, Marine Aircraft Group 39,
3rd Marine Aircraft Wing, Camp Pendleton, California.




Lake County: Remembers Our Fallen

Pfc. Ivan I. Wilson from Clearlake, California
At the age of 22 years old Pfc. Wilson was killed on July 21st, 2008 while supporting combat
operations in Helmand Province, Afghanistan. A member of the Marine 2nd Battalion, 7th Marines,
1st Marine Division, 1st Marine Expeditionary Force, Twentynine Palms, California.



Marin County: Remembers Our Fallen

Spec. Jake R. Velloza of Inverness, California
At the age of 22 years old Spec. Velloza died in Operation Iraqi Freedom of wounds sustained
after being shot by enemy forces in Mosul, Iraq.  Assigned to the Army 1st Battalion, 12th Cavalry Regiment, 3rd Brigade Combat Team, 1st Cavalry Division, Fort Hood, Texas.

Spec. Nicholas P. Olson of Novato, California
At the age of 22 years old was killed on the 18th of September, 2007 during Operation Iraqi
Freedom from injuries when a makeshift bomb detonated near his unit during combat operations
in Muqdadiyah, Iraq. Spec Olson served with the Army 2nd Battalion, 23rd Infantry Regiment,
4th Brigade, 2nd Infantry Division (Stryker Brigade Combat Team), Fort Lewis, Washington.

makeshift bomb detonated near their Humvee following a mine assessing mission in Samarra Iraq.

Combat Team, 3rd Infantry Division, Fort Stewart, Georgia.

Ubaydi, Iraq.  Lance Cpl. Lucente served in the Marine Battalion Landing Team 2nd Battalion,
1st Marine Regiment, 13th Marine Expeditionary Unit, 1st Marine Expeditionary Force,
Camp Pendleton, California.



Placer County: Remembers Our Fallen

SGT Joshua M. Hardt of Applegate California  
SGT Hardt was killed in Afghanistan at the age of 24 years old, one of Eight soldiers who were
supporting Operation Enduring Freedom. 
They died Oct. 3 in Kamdesh, Afghanistan, of wounds
suffered when enemy forces attacked their contingency
outpost with small arms fire, rocket-propelled grenades and indirect fire. They were assigned to the Army 3rd Squadron, 61st Cavalry Regiment, 4th
Brigade Combat Team, 4th Infantry Division, Fort Carson, Colorado.

wounds sustained after being shot by enemy forces in Mosul, Iraq.  Spec McCleery served with the

makeshift bomb detonated near his position in Husaybah, Iraq.  Cpl. Shea served with the
Army 3rd Battalion, 75th Ranger Regiment, Fort Benning, Georgia.



Sutter County: Remembers Our Fallen

Pfc. Daniel P. Cagle from Carson City, California
At the age of 22 years old, Pfc. Cagle was killed in Ramadi, Iraq when a makeshift bomb
exploded near his unit.  Assigned to the Army, 3rd Battalion, 69th Armor Regiment, 1st Brigade
Combat Team, 3rd Infantry Division, Fort Stewart, Georgia.




Tehama County: Remembers Our Fallen

Sgt. 1st Class Allen C. Hohnson of Los Molinos, California
At the age of 31 years old, died in Operation Enduring Freedom by small-arms fire during an attack
on his patrol in Khanaqin, Afghanistan.  Served with the Army 1st Battalion, 7th Special
Forces Group, Fort Bragg, North Carolina.

Sgt. Timothy C. Kiser from Tehama, California
Died at the age of 37 years old on April 28th, 2005 in Operation Iraqi Freedom, killed by a makeshift
bomb while on patrol in Riyhad, Iraq.  Sgt Kiser served with the Army National Guard, Army National Guard's 350th Forward Support Battalion, 40th Infantry Division, Red Bluff California.



Trinity County: Remembers Our Fallen

Sgt. Jeffrey S. Brown from Trinity Center, California
Died at the age of 25 years old on August 8th, 2006 in Operation Iraqi Freedom, died when his
UH-60 Blackhawk crashed into a lake in Rutbah, Iraq.  Sgt Brown served with the Army 82nd
Medical Company (Air Ambulance), Fort Riley, Kansas.



Yolo County: Remembers Our Fallen

Pfc. Justin A. Casillas of Dunnigan, California
Died on July 4th, 2009 in Operation Enduring Freedom when insurgents attacked the
outpost using small arms and indirect fire.  Pfc. Casillas was assigned to the Army 3rd Battalion,
509th Parachute Infantry Regiment, 4th Brigade Combat Team (Airborne), 25th Infantry
 Divination, Fort Richardson, Alaska.

Petty Officer 1st Class Neil C. Roberts from Woodland, California
Died at the age of 32 years old on March 4th, 2002 in Operation Enduring Freedom. 
Petty Officer 1st Class Roberts served in the Navy.

Staff Sgt. Jimmy J. Arroyave from Woodland, California
Died at the age of 30 years old on April 15th, 2004 in Operation Iraqi Freedom, killed in a vehicle
 accident in Ramadi, Iraq.  Staff Sgt. Arroyave served with the Marine Combat Service Support
Battalion 1, Combat Service Support Group 11, 1st Force Service Support Group, 1st Marine Expeditionary Force, Camp Pendleton, California.



Yuba County: Remembers Our Fallen

Lance Cpl. Adam J. Strain from Smartsville, California
Died at the age of 20 years old on August 3rd, 2005 in Operation Iraqi Freedom, killed by small
arms fire while conducting combat operations in Ar Ramadi, Iraq.  Lance Cpl. Strain served
with the Marine 1st Battalion, 5th Marine Regiment, 1st Marine Division, 1st Marine
Expeditionary Force, Camp Pendleton, California.
